Required documents

  1. Letter of Request (Word)
  2. Registration ID issued by the Embassy
  3. Death Certificate issued by the relevant Government Authority. This document should be certified by the consular Division of the Ministry of Foreign Affairs of the relevant Government.
  4. Application
    • Declaration of Death under section 30 (PDF) (within three months)
    • Declaration of Death under section 36 (PDF) (after three months)
  5. Birth Certificate of the deceased
  6. Passport with the copies of visa pages
  7. Relevant Fees
